Passport Renewal at DFA Cagayan de Oro City

During the typhoon Sendong, one of the most important things that I wasn't able to save was my passport.  So here I am, after few years of ignoring the importance of securing a new one, I finally decided to renew my e-passport.  

Things I did before renewing my passport:
Prepare the necessary documents needed.  In my case, I knew that aside from the birth certificate which was issued from NSO and some valid IDs,  I still  need other supporting documents for my damage passport,so I called their office to asked for the requirements.  For queries,  you may contact DFA Cagayan de Oro City Branch through these numbers (088) 8572175/ (088) 8576565.

Anyway, I was told that I need to provide an Affidavit of Mutilation, NSO birth certificate (but they didn't look for it when I was already there), a photocopy of my passport (page 1-3 and the last page) and a photocopy of my 2 valid IDs.

Aside from these requirements, I also asked my younger sister  to get me an application  form at the DFA Office when she applied for her passport.
For new applicants, you could also look at DFA website for other  requirements.

 Where is DFA CDO Office located?
DFA office here in Cagayan de Oro City is located at Limketkai DriveIt will only cost  you Php7.00 jeepney fare from Divisoria to Limketkai or from Agora Terminal to Limketkai.

Tips:
Be earlyDFA Office here in Cagayan de Oro City does not entertain appointment for application of e-Passport so you have to be there as early as possible.  And when I say early, it means 3 in the morning if you want to be part of the first batch of applicants.  Really! I'm not kidding. They only cater 250 applicants a day for regular passport (  passport that expires after 5 years).  The reason behind it, is that they only have 4 computers that could cater 250 regular passport plus unidentified  number of HAJJ passport (passport that expires after a year) applicants.  

Just imagine if you are from Zamboanga City and you are the 251th applicant on the line. Tsk! Tsk! I can already imagine the frustration on your face.

Bring all original documents you have, especially your NSO birth certificate. 

Be patient.  For new applicants, I now how hard and time consuming it was for you to  get a NSO birth certificate and a NBI clearance. This time, in processing your Passport here in DFA Cagayan de Oro City , you also need to be patient.  Remember, there are only 4 computers to scan your document, take your picture and encode your data into theor system.

Expect that not all employee will smile at you when they serve you.



Yup! There are smiley posters on the wall of the DFA Office saying "WE SERVE YOU WITH A SMILE",  but sad to say that not all of them are going to be nice with you.  Like what happened to me,  instead of a smile I received a sharp look from the girl on window 5. 

You might be wondering what happened why she did it to me.   Well, she got mad when my two valid IDs fell on the floor when she took my documents which I placed in front of her.  That was the time she gave me a look and murmur something and then she went down to pick it up.  And after it, she was very harsh.  She asked my with a tone why I didn't process my passport after Sendong and she also looked for  my certification as a Sendong victim.  Certification? wait! the last time I called DFA office and asked for the requirements they didn't said anything about a certification.  Anyway, she talked to the girl on window 4 if the affidavit which I presented was alright, good thing that girl knows better than her.  GRRR... Before she gave me my papers she told me to inform the encoder on the  e-passport to scan my affidavit  then she  told me to affix my signature on the lower part of the application form.  So I looked for my pen inside my bag, when I wasn't able to see my ballpen, I asked her if I could borrow a pen from her.  Again she replied to me with a tone, "Pula  ang ballpen ko."  (My pen is red.)  So, again I looked back to my bag and finally found my black pen. Whew!  Later I  found out that it wasn't only me who had a bad experience with the girl on window 5.  

Let me clear things up, the time I processed my passport I had a bad experience at the girl on window 5, but maybe by the time you process your own passport that girl on window 5 might be assign in other things or in other window.  So just observe the person validating your documents and fell free to share your experience here. :)


SIMPLE STEPS ON RENEWING OR APPLYING A PASSPORT AT DFA CDO:
1.  Fall in line outside the DFA Office as early as possible.
2.  Wait for the guard to get your form.
3.  Wait and listen to the guard on what time you are ask to return to DFA office to process your passport. 
4.  On your schedule time, return and wait outside the DFA office until the guard tells you to get in.
5.  Once inside the DFA office, wait for your name to be called on one of the windows that will check the documents or requirements you brought.
6.  After validation, proceed to the cashier.  DFA Cagayan de Oro does not have a express lane for passport application which is worth Php1,200.00 as of the time I process mine.
7.  Proceed to the e-passport encoding area, they have a box there where you should place your form.
8.  Wait for your name to be called by one of the e-passport encoder.
9.  Once your picture, finger prints and signature specimen have been taken.  Check carefully the print-outs that they will be giving to you.  If the details on the form are correct, affix your signature on lower part of the form.

  5. Well according to the DFA Staff, you have to bring your baby. Just inform the guard on duty that you have an infant with you and go to Window 1.

  7. I also have bad experience during my application for my passport, and yes you're correct that most of them talked "pataray ang dating".. specially those who assigned in window three and two.. I know how it feels if you have a lot of applicants to handle or process, Coz i also work w/ that kind of environment w/c u are facing people and take note irate people's.. But still we we're able to manage and maintain soft tone of our voice w/ a smile.

    U know what makes me very disappointed????? At the first day i went their presented my documents as indicated in the application form,it was noted on my application that i still need to present my TOR/MDR. I comply my lacking documents & went back in the second day, Presented back all the requirements including my old NBI as also presented in the first day, and ask them " okay lang po mam nah OLD NBI ko pah ito? she replied it's okay coz dis is only supporting documents..

    A month passed by,I went back to DFA and excited to claim my passport, Shocked :( my application still in pending status and their reason was " I NEED TO GET A NEW NBI " I admitted that i get mad but still pretending i am not, I ask them, The last time you said that my OLD NBI will do?

    I also asked them why not anyone from this office didn't informed me about this matter? A simple replied, they called and text me daw but i didn't feedback, as they claimed. WHATTTT how could i replied your text and answered your call if i haven't received any of that?????


    Pero wala nah akong magagawa!! ako kasi ang nangang-ilangan, so i will get my new NBI and wait for another days........... Huwag naman sana maulit ito sa iba, i also have complicated schedules. :(HOpe it will not happen again..

    2. I am senior. And yes, its true that load of applicants went there, running after the other, when the guard allowed us to enter. When i applied for a passport last November, 2014, my escort who was my nephew was not allowed to get inside. Just i was thankful that there was also a lady senior who was familiar to a place who guided me, for its the second time she went there, according to her. Its really difficult for the first time to get there inside. Anyway i have my all documents ready since i inquired first at the travel agency in our area for the needed documents for applying passport.

  11. Wow! This blog is very useful for me na first time kukuha ng passport one of this days. I saw an online passport appointment sa internet, filled-up all the details and got an appointment. Pero nagtataka ako kung accepted ba ang passport appointment sa DFA Cagayan De Oro and I found here na you still have to fall in line as early as possible. Im from Iligan City and I think the best way for me to be early in DFA is to stay in any Rental Room nearest the DFA office a night before that morning I planned to apply for my passport.

    Tanong ko lang po ito po ang mga ducoments na meron ako, at mga ids. Gusto ko lang po ma make sure na okay na ito at kung may kulang pa po bah.. I already have:

    Barangay Clearance
    Police Clearance
    NBI Clearance
    NSO Birth Certificate
    Certified True Copy (CTC) of BC issued by the Local Civil Registrar
    Baptismal Certificate
    SSS E-1 Form

    And My IDS are only School I.D and Postal I.D ..

    Is that all enough? Or Is there anything I have to get before going to DFA Cagayan De Oro??
    I plan to go there October 8 this year so I am hoping to receive a reply from you before that date.

  18. Hello please advise if DFA Cagayan de Oro still offer express passporting and how many days it will take in actual number of days from application to receiving the new passport. I'm going to renew my passport end week of December 2013 and I only allotted around 3 weeks before travel. Thank you very much for the info.

    1. You have to talk to the releasing officer and check if na cancel yong passport mo. If confirmed na nacancel (yong di kinuha on schedule releasing of passport considered cancel siya), you have to request for a certification of cancellation.

      And I think you have to apply for a new one. Anyway, here is the number of the DFA office for more details regarding your concern, (088) 8572175/ (088) 8576565. Baka kasi they would ask me some information about you which I can't provide.

Do you think it's okay to renew passport in Philippines, in my case, who has a status of residency in Japan? I will take vacation in our country soon and plans to do the renewal at the same time.     1. A piece of advice, it would be better if you renew your passport in Japan before going here for vacation. Why? The last time I renewed my passport, there was also a co applicant, who is already a resident of Japan by the way, who decided to renew her passport here and applied for her sons passport as well. Anyway, to make the story short, it was a very stressful day for her. She have to deal with the girl at window five, have to write and request for the head officer to allow her passport to be delivered and process before July 10, and she was able to finished the entire application process at around 7 in the evening. Once in a while she keeps on saying that she should have process her passport before going back here in the Philippines.

      But to answer your question, yes you may.

    I'm a new applicant. and wished to process and own a passport..
    I only have this -

    - NSO birth certificate
    - Voter's ID
    - Brgy. Clearance (issued April 2013)
    - Community Tax Certificate (issued April 2013)
    - SSS E1 form

    Based on my research I just have to have at least 3 of all those listed supporting documents.
    But I'm so curious about the NBI clearance? you think it is a must? or any other documents that I should have with me upon seeing their office? I'm currently at Iligan City for work based but i'm from Butuan City.

      I recommend that you bring with you a NBI Clearance especially that this is your first application. Also, it would be best if you bring other supporting documents such as TOR and Baptismal certificate cause sometimes the details of our NSO birth certificate are not that readable such as your name and birth date.

      Remember to bring the original documents.

    3. Depende po yan. katulad po ng kapatid ko, hinanapan pa po siya ng baptismal though meron napo siyang NSO Birth Certificate. Minsan po kasi di po clear ang names na nakasaad sa NSO, by the way ano po pala dala nyong documents?

I have na my School ID, NSO Birth Certificate, NBI Clearance, Barangay Clearance, Police Clearance.. San po ba pwedeng kumuha ng application form? Hindi pa ako naka pag set ng appointment though.

    3. Hello po.. I went to DFA last January 9, 2014.. I was there 4am in the morning and nag open sila by 8am.. Unfortunately, nakulang daw yung mga supporting docus ko.. I'm just confused kasi dun sa site ng DFA, sabi AT LEAST 2.. So i brought my COLLEGE ID, NSO BIRTH CERTIFICATE, NBI CLEARANCE, POLICE CLEARANCE, BAPTISMAL, and BARANGAY CLEARANCE.. I thought it was enough.. Kasi nga at least 2.. tsaka hindi nila in-accept ang BARANGAY CLEARANCE ko kasi di pa daw nag 2 years.. di ko daw binasa ng ma-igi sa site nila.. panu eh wala naman sinabi dun.. Kinda disappointing.. Nag effort pa ako mag wait ng 4 hours for nothing.. And galing pa ako ng Bukidnon.. 3 hrs byahe from CdeO.. Sana update2 din nila site nila. -_-

    5. Pinabalik talaga sa akin lahat ng papers ko. Di man lang kinonsider na ibabalik ko lang ang kulang kapag makukuha ko na Passport ko. Muntikan na akong umiyak dun. Pinadala ako nung either FORM 137, VOTER'S RECORD, MDR, TOR.. Ayun, pagbalik ko ng CDO may dala na akong FORM 137 and VOTER'S RECORD.. And at last na process din. Di na ako nag line, agad2 akong pumasok. lol. I hope na sana pag kuha ko ng Passport ko eh, walang mga kapalpakan na naman. Kasi ang hassle, 3 hours pa byahe papuntang CDO.. -_-

  33. Hello ma'am... nakakatulong po kayo ng malaki. Sana lang matulungan nyo ako. Permanent resident na po ako ng US at uuwi lang sa emergency na punta kami CDO. Taga dun po ako sa Luzon at malayo ang DFA office mula sa amin. Dun lang po ako sa CDO ng 3 days at balak ko nang mag apply dun ng passport.

    Nag asawa po ako ng foreign national, dito sa abroad at meron akong Report of Marriage. Mag change status ako ng married name, kasama ang renewal. Kailangan kong makabalik dito US in 30 days. Dahil less than 6 months ang validity ng passport ko, kailangan ko magrenew. Sinabihan po ako sa consulate na sa Philippines na lang magrenew para makabyahe ako pauwi sa Pinas. Sa CDO po, -

    1. Meron pa bang rush passport? ilang araw po ito?
    2. Kailangan pa ba akong magdala ng picture?
    3. Pwede po bang mag walk in o dapat me appointment? di po ako makatawag dito.
    3. Dahil residente na po ako sa ibang bansa, expired na po mga IDs ko. Ito po ang meron sa akin:

    1. Report of Marriage na tinatakan ng consulate ng abroad
    2. SSS ID
    3. PRC ID - expired na po/ old
    4. Driver's License - expired na po/old
    5. Postal ID - expired na/ old
    5. NSO birth certificate
    6. TOR
    7. CFO certificate and sticker

    Are these enough? pls. advise..Thank you, sana matulungan nyo ako dahil wala akong mabalingan.

      Sa manila po meron pong 7 working days
      pero for regional office such as CDO 10 working days po
      and 1200 po ang babayaran nyo po

      di na po kailangan magdala ng picture since electronic napo sila

      no appointment po sila dito sa cdo, first come first serve basis
      kailangan po kayong magline up ng maaga, kumuha na rin po kayo ng form

      dalhin niyo nalang po yan, if may id kayo issued abroad dahil nyo po
      at saka, kailangan nyo pong kumuha ng NBI clearance.

    Gusto ko po sanang kuhan ng pasport and anak kung 3 years old.. kaya lang sa birth nya Not Married pa ang nakalagay kasi naparehistro namin xa bago pa kami kinasal.. ngayon nag file na ako ng secpa para machange yung info nayon.. kaya lang tagal pa makuha after a month pa daw.. eh paano pu kilangan na ksi namon makakuha ng passport... pwede pu ba na issuport ku nlang sa not married namin na nakalagay sa nso niya ang aming marriage contract? sana po matulongan po nyo ako.. salamat..

      For Minor Applicants (below 18 years old)


      General Requirements:

      Personal appearance of the minor applicant
      Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)
      Original Birth Certificate of minor in Security Paper issued by NSO or Certified True Copy of Birth Certificate issued by the Local Civil Registrar and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. Report of Birth duly authenticated by NSO is required if minor was born abroad.
      Document of identity with photo, if minor is 8-17 years old (for first time and renewal applicant) such as School ID or Form 137 with readable dry seal
      For minor applicants who never attended school, a Notarized Affidavit of Explanation executed by either parent (if minor is a legitimate child) / by mother (if minor is an illegitimate child) detailing the reasons why the child is not in school, is required
      Marriage Certificate of minor’s parents duly authenticated by NSO
      Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child)
      Original and photocopy of valid passport of the person traveling with the minor
      Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) or identification documents (Please refer to List of Acceptable IDs)
      If minor is illegitimate but acknowledged by father: - since yan pa po ang nakalagay sa Birth Certificate nya.
      Birth certificate from NSO reflecting surname of father with Affidavit of Acknowledgement and Consent to use the surname of father

      Anyway, for further clarification you may call their office at (088) 8572175/ (088) 8576565. Para at least pagprocess mo wala ng problema.

I am planning to go there by 6th of Feb to renew my passport. atatakot lang ako pag arrive dun baka hindi na ako ma cater since 250 lang ina accept nila. I didnt go thru an agency kasi. Thank u so much in advance. And btw, yung supporting docs ko ay TOR, Police clearance, Brgy clearance, Voters certificate, Old and new cedula....those are all photocopied. I also have my original NSO, and my MDR from Philhealth... and sa IDs ay ang PRC id, TIN id, voters id and my recent work ID...okay na kaya yan? Thanks very much sa infossa post mo...

I got my passport last July 2011 and will expire on 2016 but I might go abroad 2-3 months from now and won't come back for 2 years. and with news that all mrp passport must be change into e-passport by october 2014 i'm worried baka magka problem pag-uwi ko. I from Iligan kasi kaya it's hard to go to cdo dfa just to clarfy if I have e-passport or not.

  46. your blog is very interesting since i was trying to call the dfa their numbers in limketketkai but that was almost a day long no one answering their phones since i was worried i ask somebody one of my cousins wife in cagayan to get a passport of my 2 minor son she has all the documents and money and a month ago and says wait and wait since its so difficult to make and appointment in DFA office,i even send her already and SPA on my behalf but still she said not easy to get an appointment.please any idea what should i do im not in the Phil..

    1. Online appointment will not be entertain by DFA Cagayan de Oro... she needs to line up as early as possible. Di naman kailangan siyang magline up the whole day. At around 7 or 8 in the morning ata, the guards will get your form and would tell you when you could get inside. Infants and seniors are the only one who can get inside and used their courtesy lane, they don't need to line up nah.

Hello po my question is i dont have nbi but i have old passport expired july 2013 i have cfo certificate since my husband is foreigner... this all my i have Postal id, alumni id, philhealth id, 2007 school id, 2010 red ribbon TOR, recent cedula, barangay clearance, nso birth ,certified copy marriage contract... and my 2nd question po im pregnant 2 months and im from pagadian city... do i need to line 4am... and do i need to secure ob letter or ok lng wala letter to ob doctor?     1. As far as I know, you don't need to line up na po. Secure nalang po siguro a letter from your ob.. mahirap na po.
      The last time I renewed my passport I only prepared a photocopy of my two government issued id and a of my old passport.

      if there is no birth certificate

      If born in or after 1950 (January 1, 1950 or after):

      Apply for the delayed registration of birth at the local civil registry office located at the place of birth of applicant
      Submit authenticated Birth Certificate from NSO and supporting public document/s upon the discretion of the processor or Consular Officer, with correct date and place of birth (i.e. Form 137, Voter’s Registration Record, Baptismal Certificate with readable dry seal or National Commission on Muslim Filipinos (NCMF) / Office on Muslim Affairs (OMA) Certificate with photo and readable dry seal for Muslim applicants).

    2) At least One Valid ID
    3) NSO Birth Cert

I went to DFA today for early renewal of my passport then I was shock lmao I wonder why no one there I was thinking maybe I was too early lol I was 7am at DFA limkitkai mall and I ask the guard somewhere they said they transfer just today to CENTREO AYALLA MALL oh ghost and It wil open at 10am I was really dissapoint waiting for long when my number called and show them all my documents they asking me for affidavit of explanation for early renewal so I hvnt got a change to renew my passport I got no idea where to go for that affidavit of explanation is anyone has experience like this here in CDO oh .And now my question is what are here in cdo to get affidavit of explanation and how much it coast :(im a bit strees now actually my passport wil expired nex year so I want to renew it now cuz by January I wil apply a visa for OZ

      I can't call their office sa ayala kasi wala pa akong number nila. Anyway,DFA office is open from Monday to Friday, from 10:00 a.m. to 7:00 p.m., and from 10:00 a.m. to 6:00 p.m. every Saturday.

Hi Maam Melanie, i am in Pagadian right now, my passport will expire on 2016, but the embassy has advised me to have an early renewal of passport since theyre requirinf a 15pages passport....my visa is ready fo attachment... And 45days waiting in CDO, is long and stressful for me😞.. Would u suggest MANILA, to expidite my passport, how long it takes me to wait?

    P.S i have, digitized BIR, MARRIAGE CONTRACT, and VOTERS id. I left some of my requirements in my province.. Enough ra ba ni mam Mel?

      If both parents are abroad:
      • Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion (authenticated by the nearest Philippine Embassy or Consulate General). If minor is illegitimate, mother should execute the ASC.
      • Special Power of Attorney (SPA) with an attached photocopy of either parent’s valid passport (authenticated by the nearest Philippine Embassy or Consulate General) authorizing a representative in assisting the child to apply for a passport. If minor is illegitimate, mother should execute the SPA.
      • Original and photocopy of DSWD Clearance
      • There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.
      • Proper ID of the duly authorized representative (Please refer to List of Acceptable IDs)
      If minor is illegitimate but acknowledged by father:
